Report Common IT issues:
If you face any issue with technology, you can utilize the links below to ask for IT support. Before you report an issue, make sure you look at the other sections in the left side menu ( 1:1 program services, and Common IT services) to see if what you need is covered by one of the service requests.

Applications Issues
CMSD Staff:
1- Click here to go to the issue reporting form.
2- If this is for a student account, lookup the student name in the "Submit on behalf of a student" field.
3- In the category field, expand the Application category and select an application from the list. If the application is not listed, select Other Apps - not listed.
4- Fill the rest of the fields.

Cyber Security Incidents
CMSD Staff:

2- If this is for a student account, lookup the student name in the "Submit on behalf of a student" field.

Internet & WIFI
CMSD Staff:
1- Click here to go to the issue reporting form.
2- In the category, select internet and network connectivity. If your issue is related to blocked web content, it is better to use the allow/block website service from the common IT services on the left side menu.

Passwords and Login Issues

2- In the category field, select accounts and Passwords.
3- If this is for a student account, lookup the student name in the "Submit on behalf of a student" field.
4- Fill in the rest of the fields.

Printers Issues
CMSD Staff: Click here to report an issue
Description: You can report any issues you face with printers. The form categorizes issues into 3 main categories. Try to choose the one that is closest to the issue you are facing. You can utilize the description field to further explain the issue.
Expected resolution time: a technician will handle your request within 48 hours.

Projector Maintenance
CMSD Staff: Click here to report issue
Description: use this form when a projector is not working. You can report a maximum of 5 projectors in one form. In most cases, the maintenance work will be replacing a filter or a bulb, or performing cabling work.
Expected resolution time: The maintenance team will contact the school/building within the same day to schedule maintenance. All maintenance work is finished in one day.

Device Issues - when not listed under 1:1 program services
Some issues related to devices are not covered in the 1:1 program services, or in the common services and issues. Please limit the use of the link below to the following scenarios:
1- When an iPad does not have the right apps. Be specific if it is early childhood ipad, SPED, or other use.
2- When a Chromebook does not have the right apps
3- When your issue is related to a desktop, like being slow, not powering up, or it keeps freezing.
4- When a MacBook needs to be configured to access CMSD network
5- When a desk phone is not working
