Cleveland Clinic Children's School-Based Health Care

CSMD has partnered with Cleveland Clinic Children’s School–Based Health program, which offers a mobile, full-service pediatric office staffed with healthcare professionals. Voluntary, comprehensive healthcare services are offered to students from kindergarten through 12th grade.
Students are seen for:
- Complete physical examinations
- Mental health consultations
- Immunizations and health screenings
- Routine labs
- Adolescent reproductive health care
- Cold and flu symptoms
- Simple sprains and strains
- Minor bumps and cuts
- Seasonal allergies
- Conjunctivitis (pink eye)
- Skin rashes
- Medication checks (evaluate & change, if appropriate)
- And more!
Appointment Information

- Students must be enrolled in order to be seen. Enrollment can be completed at clevelandclinicchildrens.org/regform.
- Cleveland Clinic Children’s mobile unit visits varying CMSD schools Monday through Friday from 9 a.m. to 2 p.m.
- Virtual visits are available Monday through Friday during business hours.
