As alumni engagement and legacy transition activities continue across several Cleveland Metropolitan School District high schools, many alumni, former staff, families, and community members have expressed interest in preserving pieces of their school’s history and legacy.

To help support this process, CMSD has established an official Transitioning Schools Item Interest Form for individuals interested in requesting specific items from transitioning school buildings.

If you are interested in obtaining a specific item, artifact, trophy, display piece, memorabilia item, or other keepsake from a transitioning building, please complete the official request form below:

Please note the following:

• No items are being distributed at this time.
• Submission of this form indicates interest only and does not guarantee receipt of an item.
• All inquiries and requests will be reviewed according to Cleveland Metropolitan School District Board of Education policies and procedures.
• Interested parties may be contacted at a future date with additional information regarding the review and distribution process.
• If requesting multiple items, please submit a separate form for each item.
• It is highly recommended that you include a photo of the requested item whenever possible to assist with identification and review.

Need a copy of your transcript or graduation verification?

The Cleveland Metropolitan School District maintains archived records for former CMSD students and alumni in accordance with District policy and FERPA guidelines.

• Current CMSD students should contact their school counselor directly.

• Requests for records from closed schools should also be submitted through the official request form.

• Processing times may vary depending on the age and availability of records.
