To enter a request to be a parent/family member volunteer at your child's school, please click here. This form is offered in English and in Spanish.

This form is only for parents and families of CMSD students who wish to volunteer at their child's school and are over 18 yrs. Please complete all sections, the Volunteer Request form is your request to serve with CMSD.

Once the school administrator accepts your intent of service, you will receive a link via email to complete your Volunteer Registration and security clearance information. Please expect a response within 5 to 10 business days from the time of submission.

Step 1: Individuals, not with a Community Partner Group (view below), must complete the electronic Volunteer Request Form (English – Español). The volunteer request form is a person’s request to serve with CMSD.

Step 2: For returning volunteers, contact information will need to be updated if changes have occurred (i.e., address, telephone numbers, email address, and emergency contact information). Please email judith.lozada@clevelandmetroschools.org with updates.

Step 3: Principals and/or a designee (Volunteer Lead); and/or CMSD Department Staff working with volunteers, will follow up with the request and confirm (or deny) with the Volunteer Office the volunteer request for service.

Step 4: Once the school administrator or designee confirms the request, volunteers will receive an email with registration and security check instructions.

If you have not received an email with registration and security instructions, this may be due to the following:

• The school administrator or school volunteer coordinator has not yet confirmed your request. If school has not contacted you, please call the school for information.

• Volunteer email was provided with a typo.

• Volunteer email may have gone to the volunteer's spam folder.

• If unsure, please contact Judith.Lozada@clevelandmetroschools.org for assistance.

To volunteer as a coach support, you must have completed the following steps:

Step #1:  Please ask the School Principal of the building you wish to serve to confirm via email with the district Manager of Volunteers your service at this building. Email to: Judith.lozada@clevelandmetroschools.org.

Step #2:  FIRST AID FOR COACHES Permit holders fulfill this requirement through ONE of the following four options:

1. Ohio Department of Education-approved pupil activity program:

• Search for a program here.
• Districts may verify completion of training by clicking on the Pupil Activity link on the CORE Dashboard and choosing the Find Participants drop-down menu.

2. Nationally approved programs:

1. National Federation of State High School Associations & American Red Cross:

• “First Aid, Health and Safety” (online)
• Districts may verify training online here.

2. Human Kinetics Coach Education Center

• “Sport First Aid” (online)
• Districts may verify training online here.

3. American Heart Association

• First Aid course
• Online training verification is not available.

3. College or university courses:

• Courses must be completed within the past three years and be related to first aid or athletic training.
• Online training verification is not available.

4. Medical licenses

• The licenses MD, DO, DC, PA, RN, EMR, EMT, AEMT, Paramedic, ATC and SCS fulfill the first aid training requirements (permit holder’s board license must be active).
• Districts may verify Ohio board licenses online here.

Step #3: Concussion Management Certification: 

Permit holders must complete ONE of the following free, online trainings every three years. Please see the Ohio Department of Health’s website for additional resources and information.

2. National Alliance for Youth Sports Concussion Training

• “Concussion Training”

3. National Center for Disease Control and Prevention Program

• “Heads Up to Youth Sports: Online Training”

• Online training verification is not available.

Step #4: SUDDEN CARDIAC ARREST TRAINING

• Permit holders must review the online video and related fact sheet approved by the Ohio Department of Health annually.
• Online training verification is not available.
• To confirm you have read and understood the online training and reviewed the law please sign and have your school Principal and Athletic Coach sign HERE.

Step #5: Current FBI/BCI,

The security check must be registered with the Ohio Department of Education (ODE), provided through the Department of Safety and Security, East Professional Center, 1349 E. 79^th Street. Cost:  $47.25.

Additional information:

• • If you are a certificated teacher, your background check is renewed along with your teacher certificate.
  • If you are a current CMSD staff member, please provide us with your staff ID #.
  • For all others, your background check must be done within the same year that you are applying to volunteer.

Note: PROVIDE COPIES OF CERTIFICATIONS AND REMEMBER TO KEEP COPIES OF ALL THE ORIGINAL CERTIFICATIONS IN A SAFE PLACE OR FILE.
