You must use this link (CMSD ONLINE ENROLLMENT) to create your account and submit your documents.
Be sure you have the following documents ready:
· Birth Certificate
· Immunization Record
· Proof of Residency (i.e. Lease, Mortgage Statement, Bank Statement, Pay Stub)
· Parent/Guardian Identification
· School Records (Withdrawal Form, most recent Report Card, or Transcript)
· Guardianship/Custody Documents (if applicable)
· IEP, ETR, or 504 Plan (if applicable)
