Health Service Consent Form
Remote School Scholars now have access to the Metro Health Mobile Unit here at John Adams! Through Metro Health’s School Health Program, children are given access to the same basic care services that they would get at the doctor's office. If the family doesn't have a primary care doctor for their child, that role can be filled through this program. If he or she already has a primary care doctor, the program can be used as a backup when something comes up and they aren't able to make it to the doctor's office.
Below are the services offered:
Well-child visits (annual physicals)
Regular physical exams for school, sports, work or camp
Immunizations
Care for childhood illness and urgent care visits
Care for common concerns like acne, menstrual or weight issues
Mental and behavioral health screenings and referrals to in-school services
Basic lab testing

The attached form will need to be completed in order to take advantage of this program.
