Hello CIS Plus families! 

We are looking forward to having your child enrolled with CIS Plus this school year! Attached to this email please find the program handbook; authorization to pick up; emergency contact form; medical form; and authorization to administer medication. Please complete and return all 4 forms for each child via email to this email address. A $25 seat deposit per child is due by August 1 to secure your child's spot. This deposit will be applied to the remaining fees for the month of August. The remaining balance for the month of August will be billed on August 1 and is due by August 5.
