These forms are required IN ADDITION to the OHSAA Pre-Participation forms for students who want to participate in the Crew / Rowing Team and activities.

Page 1 must be filled out by a certified lifeguard or American Red Cross Safety Instructor and validates that you are able to swim.

Page 2 must be filled out and signed by the student and caregiver.

This PDF includes all pre-participation forms needed to participate in any athletic program at Campus International's Upper School.

Pages 1-4 must be filled out, signed, and stamped by a physician or medical examiner.

Pages 5-10 must be read and signed by students and their caregivers.
