• TRANSCRIPT AND RECORDS REQUEST


    Cleveland Metropolitan School District (CMSD) is responsible for the archiving of student permanent records of former and alumni CMSD students.  

    Per District policy and the Family Educational Rights and Privacy Act (FERPA), student records (graduation verifications, transcripts, immunization records, etc.) are only available to those who have authorization from the student or parent, guardian, or responsible person if the student is under 18 years of age. If the student is 18 years of age or older, school records will not be released to a parent, guardian or responsible person without the student’s written consent.

    If you are a current student, currently enrolled in a CMSD school and need a transcript, contact your school counselor. If you have withdrawn from CMSD at any time during the current school year, contact the last school attended for copies of records. If the school you or your child attended is still an active and open school, contact the school directly.

    School names or school contact information may have slightly changed since you last attended. Visit our School Directory for an up-to-date list of open schools, addresses and hours.

    Important Information:

    • For Cleveland residents that attended a charter/community school that is now closed, submit your request for records using this form REQUEST FOR TRANSCRIPT (for Spanish click here, Español).
    • For those requesting to pick up records, an official, unexpired photo ID is required, such as a driver's license, passport or military ID.
    • Please allow 10-15 business days to process student records orders. 
    • For a record request from 1969 or earlier, it will take longer than 10-15 business days due to 3-party inaccessibility. These records are not digitized and are kept off-site in a secure, controlled environment.

     

    All requests will be checked for accuracy. Any inaccurate information may result in your record not being processed. All communication regarding your request will be sent via email.
