Service requests related to 1:1 technology ( staff devices, student devices, and accessories) are under the 1:1 program section.

Allow or block a website / online content
CMSD Staff: Click here to request service
Description of Service: Many online resources, like websites and videos, are blocked to ensure the safety of students. As online content constantly changes, educational resources might be unnecessarily blocked. Use this service to report online content that is blocked, but shouldn't be, or vice versa.
Instructions: Make sure to identify the online content in the request form ( e.g. a website url). Also, indicate the reason why that content should be allowed or blocked.
Turn Around Time: 1-2 days

New CMSD Desk Phone

Description of Service: You can use the form to request one or more new desk phones.
Turn Around Time: 5 days

New CMSD Mobile Phone

Description of Service: District issued devices can be ordered using the link above. In the form, make sure to select the right school / building and office/room #, if you choose for the device to be sent to you

Description of Service: IT set up a shared outlook address for teams who need that.

Request an Account for a non-CMSD employee
CMSD Staff: Click here to request service
Description of service: This service allows a vendor or a contractor to have a CMSD email address, and/or access to CMSD resources.
Approval note: when a request is submitted, an email will be sent to the supervisor that manages the contractor/vendor asking for their approval. The supervisor can approve/disapprove by responding to the email with Yes/no or approved/ disapproved.

Training Consultation

Definition of service: IT provides training and consultation around using common applications and technology equipment. There is a host of ways the technology training team can provide support, including in-house training, coordinating training from third parties, virtual and in-person training sessions, and developing training content like videos and documents.
Turn Around Time: The technology training team is committed to responding to any request within 1 day. The time to schedule training activities can vary, and will be coordinated with schools.
