MetroHealth School Health Program
CMSD has partnered with MetroHealth to offer basic care services at your school that your child would receive at the doctor's office. The clinic is open to all students from the school as long as there is a signed consent form on file. There is no cost to families or students to come to a MetroHealth School Health Program Clinic. In fact, you don't have to be a current MetroHealth patient.
Services offered by the Clinic:
- Sports/school/work physicals
- Immunizations/shots
- Urgent care visits
- Care for common health concerns (asthma, diabetes, etc.)
- Well child visits
- Teen health issues
- Mental and behavioral health screenings and referral to in-school services
- Basic lab testing

Elementary Schools
- Buhrer Dual Language Academy
- Fullerton
- Miles Park
- Mound STEM
- Scranton
High Schools- Garrett Morgan High School
- John Adams High School
- Lincoln-West High School
Immunizations are also being administered at 5 other school location:
- Anton Grdina
- Harvey Rice
- Luis Munoz Marin
- Walton
- Willow
Parents will be sent home a letter and consent packet concerning the immunization clinic days.How to use our program:
1. Register/Consent- Complete Patient Registration Form
- Sign the Consent Form 2 times, front and back
2. Return Forms- Return both signed forms to school nurse
3. Payment- NO cost to families; NO copays
- Insurance will be billed, when able
4. Schedule appointment- Contact school nurse to schedule an appointment for child to be seen
5. Clinic Visit- Parents can come, but don't have to be present for clinic visit
- Parent can be called on the phone during visit
- After Visit Summary sent home
