• Secondary and Postsecondary Transition

    Office of Transition Services

    CMSD has a team of Special Education Transition Coordinators who can provide the following services and supports for students with disabilities ages 14-21, their families, and their school teams:

    • Participate in the development of IEP/Transition Plans and document transition activities to be provided
    • Serve as a resource to parents, students, and school personnel by providing information on transition planning and how to access transition services
    • Consult/Collaborate with Intervention Specialists, General Education Teachers, Career Tech. Ed. Teachers, Guidance Counselors, Administrators, other Related Service Providers, and Stakeholders as it relates to the transition process for CMSD students in person or remotely
    • Provide professional development formally or informally to staff on transition topics, transition age IEP’s, and changes that occur in special education laws specific to the transition process
    • In collaboration with Intervention Specialists, provide and/or administer age appropriate transition assessments (AATA’s) to help identify students’ preferences, interests, needs, and strengths
    • Support Intervention Specialists in planning pre-vocational instruction, career exploration, and travel training
    • Complete the referral process to the appropriate State, County Board, or other adult services providers for students requiring those services, and assist the family and Intervention Specialist in interpreting evaluation results
    • Assist Intervention Specialists with pre-vocational instruction prior to a community-based work experience (CBWE)
    • Establish contact with appropriate community employers to develop specific job training and placement sites for students with disabilities
    • Facilitate agreements between the appropriate District personnel, agencies and/or local businesses to outline specific responsibilities for the students, and develop annual CBWE signed contracts between CMSD and the same
    • Serve as a resource to parents, students, school personnel, and additional stakeholders by providing information on high school choices and access to secondary and post-secondary transition services
    • Maintain hard copy and electronic records of appropriate documents during the transition process
    • Identify, establish, and maintain linkages with community resources (e.g. Ohio Means Jobs, Project Search, Say Yes to Education) and local businesses
    • Develop linkages with other agencies to ensure transition to adult services (including supported/integrated employment, independent living, and community participation)
    • Collaborate and plan with Intervention Specialists or other school staff on employability skills training and/or instruction, appropriate job placements, and referrals to post-secondary education and training
    • Provide assistance with instructional strategies, including accommodations and modifications for students with disabilities in all CMSD education programs
