Policy
Health Records
The District requires health records of students under the following circumstances:
- Kindergarten and first grade students entering school for the first time must have a completed health record before being admitted to a school. The record must include information on the medical screening tests required by law.
- Any student transferring into the District must provide health records. If the previous school does not forward a record, or if it is incomplete, it is the parent or guardian's responsibility to comply with health requirements for students.
- Students must have physical examinations before they participate in interscholastic athletic programs.
Screening
The District screens students enrolled for the first time in kindergarten or first grade for hearing, vision, speech and communications, health or medical problems, and any developmental disorders prior to November 1 of the school year. The District notifies parents before August 1 of the screening year and allows parents to submit a written statement excluding their children. If the results of any screening reveal the possibility of special learning needs, the District conducts a further assessment following State law.
The District reports compliance with these screening requirements to the Ohio Department of Education by November 30 annually through the consolidated school mandate report.
If the District reports non-compliance, the Chief Executive Officer or designee provides a written explanation to the Board within 30 days explaining this non-compliance and a written plan of action for accurately and efficiently addressing the problem.
Invasive physical examination means any medical examination involving the exposure of private body parts or any act during such examination that includes incision, insertion, or injection into the body. This policy does not apply to a hearing, vision, or scoliosis screening. In addition, this policy does not apply to any physical examination or screening permitted or required by state law, including physical examinations or screenings permitted without parental notification.
The District annually notifies parents of the administration of health and physical screenings and examinations and allows parents to exclude their children.
